The best parking in Old San Juan is in one of the two garages on Calle Recinto Sur, either Dona Fela Parking garage or Galeria Paseo Portuario garage.
The streets are cobblestone and very uneven! Also, Old San Juan has many hills.
The area is generally safe but be aware it is a known tourist area. There is an area on the north wall of Old San Juan called La Perla. It is featured in the Despacito Video but is not known as a safe or good area. There's not much to see there that you can't view from the wall of Old San Juan.
